Insurance & Financial Policy
At Lone Star Orofacial Pain & Dental Sleep Medicine, our practice is dedicated exclusively to the evaluation and management of orofacial pain and temporomandibular disorders. Due to the specialized nature of care we provide, our office operates as a fee-for-service practice.
Fee-for-Service & Out-of-Network Status
Lone Star Orofacial Pain & Dental Sleep Medicine is not in network with any dental or medical insurance plans. We do not have contractual relationships with insurance carriers and are considered an out-of-network provider for all plans.
Payment for services is due in full at the time of the visit, regardless of insurance coverage. Fees are determined based on the time, expertise, and complexity required for specialty evaluation and management.
Insurance Benefits & Reimbursement
Insurance coverage is a contract between the patient and their insurance carrier. While some plans may offer out-of-network benefits, reimbursement is not guaranteed and varies widely by plan.
As a courtesy to our patients, our office may:
- Verify insurance benefits prior to your appointment when information is provided in advance
- Provide detailed documentation and coding to assist with reimbursement
- Submit insurance claims on your behalf, when applicable, as a courtesy service
Please note:
- Benefit verification is not a guarantee of payment
- Coverage determinations are made solely by the insurance carrier
- Our office does not participate in insurance appeals or benefit disputes
- Any reimbursement issued by the insurance carrier is based on your individual plan and is typically paid directly to the patient
Courtesy Claim Submission
When requested, our office may submit claims to your insurance carrier as a courtesy only. This service is provided to assist patients but does not alter our out-of-network status.
- Submission of a claim does not imply network participation
- Our office does not accept responsibility for claim processing delays, denials, or reimbursement amounts
- We do not guarantee reimbursement and do not track or follow up on claims once submitted
Patients remain fully responsible for all charges incurred.
Payment Options & Financial Arrangements
We understand that specialty care is an investment in your health. Payment plans may be available for qualifying patients and certain services.

Financial Responsibility
By receiving care at Lone Star Orofacial Pain & Dental Sleep Medicine, patients acknowledge and agree that:
- All professional fees are the patient’s responsibility
- Insurance reimbursement, if any, is determined solely by the insurance carrier
- Our practice does not waive, discount, or adjust fees based on insurance determinations
